Job Description

Autumn Lake Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN/LVN) for a 14-week contract assignment at its Crystal Springs location. The ideal candidate will provide quality nursing care, monitor residents’ health conditions, administer medications within the scope of licensure, and support the healthcare team in maintaining a safe and comfortable environment.

Key Responsibilities

• Provide direct nursing care to residents according to established care plans and facility policies.

• Monitor and document vital signs, health status, and changes in residents’ conditions.

• Administer medications and treatments in accordance with licensure and facility protocols.

• Assist residents with daily living activities and other nursing care needs as required.

• Maintain accurate and timely clinical documentation.

• Communicate with physicians, registered nurses, residents, and other healthcare team members.

• Follow infection control procedures and patient safety standards.

• Report changes in residents’ conditions promptly to the appropriate clinical staff.

Qualifications

• Active and valid LPN/LVN license.

• Previous nursing or long-term care experience preferred.

• Strong clinical, communication, and documentation skills.

• Ability to work 12-hour night shifts.

• Availability to work every other weekend.

• Commitment to resident safety and quality care.
